    Executive Summary

This executive summary presents an analysis of community health needs in Jordan, located in Minnesota. Utilizing a virtual windshield survey, we examined various aspects of this small town, including general conditions, public health needs, and environmental factors. The following executive summary provides a concise and structured analysis of the community health status in Jordan, highlighting health needs that warrant further attention for the overall well-being of the community.

General Condition and Public Health Needs

The virtual windshield survey revealed diverse aspects of the City of Jordan, encompassing educational systems, libraries, health services, recreation centers, churches, parks, residential areas, shopping plazas, and grocery stores. The city demonstrated daily thriving, particularly in supporting education through the development of elementary and high schools. Additionally, the presence of St. Francis Health Services, just a ten-minute drive away, along with Fairview Hospital and Clinics, fifteen minutes away, effectively addresses the health needs of Jordan’s entire population and ensures safety.

The City of Jordan also boasts middle and high schools, accessible libraries, fire and police departments, and recreational parks. Despite steady growth, the town faces increasing demands for educational, healthcare, and public utility systems. Enhanced access to restaurants raises concerns about public health vulnerability to diseases such as obesity, diabetes, and cardiovascular problems.

Limitations of the survey included inadequate resources, time constraints, and community members’ reluctance to communicate openly, necessitating further research through ancillary data for a comprehensive understanding of health needs and available facilities.

    Environmental Analysis

    Social Determinants of Health

Environmental factors, including population, educational and healthcare facilities, recreational spaces, and parks, significantly influence population health and community well-being. The windshield analysis identified Jordan as a small town with 7,000 residents, anticipating an annual growth of 150 individuals. The city’s robust educational system contributes to health literacy, with nearly 70% of ISD #717 graduates pursuing post-secondary education.

Adequate access to healthcare organizations, represented by hospitals like St. Francis Health Services, Fairview Hospital, and Clinics, positively impacts population health and life expectancy. Opportunities for improvement lie in utilizing available industrial park space to enhance infrastructure for healthcare and education services.

Health-promoting environments, including parks and gyms, contribute to healthy habits, while recreational places like the Minnesota River Valley positively affect physical and mental health. Churches in Jordan reflect religious concerns, indirectly influencing the spiritual and mental health of the community. Challenges include the steady population growth with limited health and educational amenities, emphasizing the need for expanded services.

Lack of adequate transportation poses a health threat in emergency cases, highlighting the significant role of social determinants in overall community health. Uncertainties revolve around understanding population socioeconomic status and social norms contributing to health behaviors.

Survey findings indicate that the City of Jordan is a well-nourished small town in Minnesota, offering basic facilities for education, healthcare, recreation, and more. However, the growing population with limited resources poses health challenges, necessitating opportunities for service expansion through improved infrastructure and sustainable growth management.
