This change proposal aims to enhance mental health among children and young adolescents, expand health insurance coverage, and improve the management of chronic diseases for low-income individuals by leveraging governmental policies, such as providing free treatment for individuals with pre-existing conditions like chronic health diseases.

    An environmental analysis of the Jordan community was conducted through a windshield survey, providing insights into both positive aspects and areas requiring improvement.

    Positive Features of Jordan Community:

    • The town offers basic amenities, enabling access to education for children, recreational activities, and healthcare services.
    • Community members exhibit resilience and a collaborative spirit in addressing health issues, fostering a strong bond with the neighborhood.

    Areas for Improvements:

    • Mental health issues among school students, leading to a tragic incident of suicide, necessitate focused attention.
    • Insufficient health insurance coverage and financial challenges in managing chronic conditions are significant health concerns requiring intricate solutions.

    Social Determinants Impacting Community Health

    Various social determinants, particularly education and economic instability, significantly impact community health. While schools exist for the community’s youth, the lack of health literacy in educational systems affects students’ mental health. Economic instability further hinders access to essential health services, resulting in limited health insurance coverage and reliance on self-financing for treatments. Our change proposal addresses these social determinants through appropriate resource allocation and support from local health advocates.

    Change Proposal Framework

    Our change proposal is structured on the following framework to address the lacking areas in the Jordan community’s health:

    1. Conducting interaction-based mental health programs in schools to provide support for affected students.
    2. Advocating for health insurance improvement plans, including expanded access to Medicaid and Medicare services.
    3. Empowering the population to acquire health insurance coverage by enrolling in ACA’s policy for treating patients with pre-existing and chronic health conditions.

    The goals encompass reducing suicide attempts among children and young adolescents, increasing the population with complete health insurance coverage, and enhancing community members’ knowledge of ACA policy for chronic diseases to improve health outcomes.

    Rationale for Policy and Financial Support

    The Jordan community requires a comprehensive change proposal to address emerging health issues, such as mental health, inadequate health insurance coverage, and poor chronic disease management. Achieving proposed changes necessitates governmental policies and financial assistance to ensure positive and systemic transformations. The Affordable Care Act (ACA) policy, with expanded Medicaid and Medicare access, is crucial for providing equitable healthcare access for the community. This policy addresses the lack of health insurance among deserving populations, promoting equitable health access for all community members (Castle et al., 2023). Furthermore, the ACA’s policy prohibiting discrimination or high premium rates for individuals with pre-existing conditions is vital for ensuring access to health insurance without financial barriers (Soni et al., 2020).

    Conducting mental health programs in schools and implementing proposed changes require both policy and financial support. Policy support aligns with governmental health missions, and financial support is essential for recruiting and training staff, obtaining materials, and covering maintenance costs.

    Evidence for Proposed Changes

    Our proposed systemic changes are substantiated by evidence-based research studies:

    1. Mental health issues in schools are prevalent globally, with 81% of teenagers experiencing problems during the COVID-19 pandemic (Rao & Rao, 2021). Interaction-based mental health programs in schools, supported by mental health specialists, have proven effective in addressing these challenges (García-Carrión et al., 2019).
    2. Lack of healthcare insurance is a significant challenge, and our proposed strategy of accessing expanded Medicare and Medicaid health policies can improve health outcomes and reduce mortality rates (Martin et al., 2021).
    3. Educating the population on governmental policies like ACA has proven effective in supporting chronic disease management (Myerson & Crawford, 2020).

    These evidence-based strategies form the basis of our proposed changes, offering valuable insights for community leaders and organizational stakeholders to implement effective solutions.

    Broad Budget Estimates

    To implement our change proposal effectively, we have outlined broad budget estimates covering human and financial resources:

    1. First-year operational cost: $520,000.
    2. Startup costs: $24,000.

    Human resource allocations include:

    • Project manager: $60,000.
    • Support staff: $80,000.
    • Mental health specialists and health professionals: $90,000 each.

    Additional resources, such as educational materials and equipment, constitute $50,000 yearly expenses. Health insurance coverage for staff members is estimated at $25,000 annually. Consultations and contract services, travel, and contingency account for the remaining expenses.

    Plan for Transformational, Evidence-Based Change

    Our vision for change focuses on improving health across all age groups in the Jordan community. The proposed changes will be implemented using Prosci’s Three-Phase process, integrating the ADKAR change model:

    1. Preparing for Change: Raising awareness and creating a desire for change.
    2. Managing the Change: Implementing required changes collaboratively.
    3. Reinforcing Change: Evaluating, reinforcing, and sustaining changes, promoting continuous improvements.

    Communication through social media and communication groups will ensure effective coordination among team members. Project managers will play a key role in directing and organizing the proposed change.

    Vision for Future Wellness, Health, and Role of Visionary Leadership

    Our vision aligns with Healthy People 2030 objectives, aiming to enhance health outcomes and healthcare access for all populations. To realize this vision, transformational leadership is crucial. This leadership style promotes visionary communication, empowerment, collaboration, and resilience, fostering an environment conducive to achieving health and wellness goals (Rubim et al., 2020).

    Cultivating Stakeholder Interest and Support

    Stakeholder participation is pivotal in achieving health and wellness goals. By addressing stakeholders’ concerns about finances, involvement in decision-making, and communication, we create a collaborative environment. Stakeholders will see a return on investment, improved revenue, and enhanced patient affordability. Early and continuous engagement, along with transparent communication, will ensure stakeholder interest and support.


    In conclusion, improving the health of the Jordan community necessitates collaborative efforts from various stakeholders.

    The proposed change, rooted in theory and evidence, has the potential to achieve meaningful goals. Broad budget estimates, a well-structured change proposal framework, and evidence-based strategies provide a comprehensive plan. Our vision aligns with Healthy People 2030 objectives, emphasizing the importance of transformational leadership and stakeholder collaboration.
